We’re seeking a Clinical Liaison (CL) – a vital connector between our senior living communities, families, care teams, and medical and hospice providers. In this impactful role, you’ll guide residents and their loved ones through critical healthcare transitions and ensure they feel supported every step of the way.
Be the go-to expert and compassionate resource for families, wellness directors, and medical providers.
Coordinate seamless transitions from hospitals to home or hospice care.
Provide clear education, guidance, and emotional support to patients and their families.
Advocate fiercely for patient needs and preferences.
Keep meticulous records and ensure compliance with healthcare regulations.
Partner with wellness directors to organize medical appointments and services.
A licensed LVN (required) or RN (preferred) with a heart for service.
Experienced in senior care, hospice, or healthcare coordination.
A clear communicator who builds trust and rapport across diverse teams.
Highly organized, proactive, and able to navigate complex systems.
Dedicated to maintaining the dignity and well-being of every resident.
LVN license required (RN strongly preferred)
High school diploma or equivalent
Experience with operations and compliance is a big plus
Skilled in HIPAA regulations and best practices
Your effectiveness in managing transitions and care plans
Strong communication and collaboration with all stakeholders
Consistent professionalism and reliability
A dynamic mix of sitting, walking, standing, and lifting (up to 50 lbs)
