The ideal candidate will be responsible for overseeing admissions to the Rehab Department and ensuring smooth, timely, and coordinated transitions from acute care settings. This role serves as a vital connector between hospitals, patients, families, and our community.
Key Responsibilities:
- Evaluate and determine appropriateness of admissions for the Rehab Department based on medical necessity, social circumstances, and insurance coverage.
- Conduct proactive rounding and on-site visits at targeted acute care hospital sites to identify potential patients and build referral relationships.
- Collaborate with patients and their representatives, physicians, hospital discharge planners, and facility staff to ensure a seamless and person-centered transfer process.
- Complete and document comprehensive pre-admission assessments, identifying clinical and psychosocial factors, and verifying financial and insurance eligibility.
- Serve as an advocate for patient needs, ensuring their goals, preferences, and concerns are addressed throughout the admission process.
- Maintain clear, timely, and professional communication with all stakeholders to facilitate continuity of care.
