Gallagher Home Health and Hospice is expanding and hiring an LPN or RN Clinical Education Coordinator with experience in Home Health and Hospice.
Position Overview
The Nurse Educator is a key clinical leadership role responsible for the development, coordination, and delivery of educational programs that support the growth, competency, and ongoing professional development of clinical staff across both Home Health and Hospice service lines. This position ensures that all orientation, training, and competency programs align with regulatory standards, accreditation requirements, and Gallagherβs mission of delivering exceptional, compassionate care.
Primary Responsibilities
Orientation & Training
- Design, implement, and lead orientation programs for new Home Health and Hospice clinical staff.
- Develop structured onboarding plans that ensure staff competency in policies, procedures, clinical documentation, and patient care standards.
- Facilitate in-person, virtual, and field-based training sessions using adult learning principles and evidence-based educational strategies.
Ongoing Competency & Professional Development
- Coordinate annual and ongoing competency evaluations for clinical staff.
- Conduct field ride-alongs with nurses, therapists, and aides to directly observe care delivery, provide real-time feedback, and reinforce clinical standards.
- Identify knowledge gaps and develop targeted training programs to support clinical excellence.
- Provide coaching and mentorship to nurses, therapists, and other interdisciplinary team members.
- Ensure staff maintain current licensure and meet required continuing education standards.
Quality & Compliance (QAPI Functions)
- Collaborate with leadership on the Quality Assessment and Performance Improvement (QAPI) program.
- Collect, analyze, and interpret quality data to identify opportunities for performance improvement.
- Develop and deliver education to address QAPI findings, ensuring compliance with state, federal, and accrediting body requirements.
- Partner with clinical managers to implement corrective action plans when needed.
Program Development & Leadership
- Stay current with best practices, regulatory changes, and clinical advancements in Home Health and Hospice care.
- Create and maintain educational resources, training manuals, and competency checklists.
- Act as a role model for clinical excellence, ethical practice, and compassionate care delivery.
- Foster a culture of continuous learning and professional growth within the organization.
Qualifications
- :
- Registered Nurse (RN) with an active state license.
- Minimum of 3β5 years of nursing experience in Home Health and/or Hospice care.
- Prior experience in staff education, training, or leadership.
- Strong knowledge of CMS Conditions of Participation and accreditation standards (e.g., CHAP, Joint Commission).
- Excellent communication, presentation, and organizational skills.
- Preferred:
- Bachelor of Science in Nursing (BSN) or higher.
- Certified Hospice and Palliative Nurse (CHPN) or Home Health specialty certification.
- Experience in QAPI program management.
