As a Behavioral Health Case Manager at Noble, you will:
What You'll Do:
Provide case management and follow-up care to support patients in meeting health and treatment goals.
Collaborate with primary care and behavioral health providers to coordinate care, treatment plans, and patient education related to mental health, SUD, and psychosocial needs.
Serve as a liaison across the integrated care team to improve communication and coordination of services.
Assess Social Determinants of Health (SDoH) and connect patients to appropriate internal and community resources.
Advocate for patients to help navigate systems and secure needed services.
Support patient re-engagement and care coordination activities.
Conduct secondary suicide risk assessments, develop safety plans, and assist with crisis de-escalation using trauma-informed approaches.
Facilitate integrated care clinical consult team meetings.
What We're Looking For:
Master’s degree in Social Work, or a related behavioral health field from an accredited program, preferred; Bachelor's degree in social work, with experience in case management or social services required.
Licensure Requirements:
Current Wisconsin licensure as a Certified Social Worker (CSW) or Advanced Practice Social Worker (APSW), or eligibility to obtain licensure within six (6) months of hire.
License must be active and in good standing with the State of Wisconsin.
