We are seeking a Utilization Review Care Coordinator. This role supports Utilization Management (UM) Department by aiding in discharge planning and coordination to ensure continuity of care following crisis placements; and provides oversight of person-centered plans to improve quality of care and ensure plans are individualized to meet the needs of each client.
This position is scheduled Monday through Friday, 8:00 am to 4:30 pm. There are no weekends, on call, or holiday requirements for this position. While the position is remote, the person in this position will occasionally travel to offsite areas such as hospitals, emergency rooms, etc.
