What You'll Do
- Serve as a clinical liaison and trusted partner to physicians, hospitals, facilities, and community referral partners
- Support appropriate referrals to Andwell’s home health, hospice, and palliative care services through clinical consultation and education
- Build and maintain strong, relationship-based partnerships grounded in trust, credibility, and clinical expertise
- Provide clinical guidance on patient appropriateness, care transitions, eligibility, and service options
- Collaborate closely with internal admissions, clinical, and care coordination teams to support smooth referral-to-admission processes
- Meet defined territory referral, service mix, and growth goals using clinically appropriate outreach strategies
- Develop and execute territory plans focused on relationship development, service awareness, and continuity of care
- Educate providers, patients, and families about Andwell services through in-services, meetings, and community outreach
- Track referral activity, maintain accurate CRM documentation, and monitor market trends within assigned territory
- Maintain current knowledge of Andwell services, regulatory requirements, and care delivery models
- Uphold patient confidentiality and comply with all regulatory and organizational standard
What You'll Bring
- Current Registered Nurse (RN) license
- Graduate of an accredited school of nursing; Bachelor’s degree in Nursing preferred
- Minimum of two (2) years of recent direct care nursing experience (SNF, LTC, hospice, or similar settings)
- Strong clinical assessment skills with the ability to translate patient needs into appropriate care recommendations
- Experience collaborating with physicians, discharge planners, social workers, and interdisciplinary teams
- Consultative communication style with the ability to educate partners on services, eligibility, and care pathways
- Relationship-focused mindset with a passion for supporting patients and families through transitions of care
- Strong organizational skills with the ability to work independently and manage territory-based responsibilities
- Proficiency with Microsoft Office and CRM platforms
- Integrity, adaptability, sound clinical judgment, and a collaborative team-oriented approach
