Position Details:
Experience working in an ADCES Accredited Diabetes Education Program providing one-on-one diabetes and nutrition education
Assists in developing core curriculum and patient materials
Provides one-on-one education sessions to teach diabetic patients about diabetes and how diet impacts their condition.
Ensures meal plans are affordable and accessible with easy to understand recipes
Monitors patient progress, adjusting meal plans and education as needed, and providing ongoing support and encouragement through follow up visits.
Uses a variety of teaching strategies, approaches, and methods that incorporate theories and concepts that are sensitive to the learning preferences of the individual, reading level, readiness for change, empowerment and motivational interviewing, and local cultural values.
Interprets personal health data, develops an individualized care plan based on the patient’s assessed needs, goals and cultural beliefs and promotes successful self-management.
Registered Dietician (RD) or Registered Dietician Nutritionist (RDN)
Certified Diabetes Care and Education Specialist (CDCES) or Board Certified in Advanced Diabetes Management (BC-ADM)
Eligible for State of Kentucky issued Licensed Diabetes Educator (LDE or MLDE)
