Description

Job Summary:
 

The Quality Improvement Coordinator supports the organization’s mission to deliver high-quality, patient-centered care by coordinating quality improvement activities and engaging patients in preventive and chronic care initiatives. This role focuses on both data-driven quality improvement and direct patient outreach to improve completion of key clinical quality measures such as cancer screenings, immunizations, and chronic disease follow-up.

Key Responsibilities:

  • Coordinate and implement quality improvement (QI) projects to improve performance on clinical quality measures and patient outcomes.
  • Conduct proactive patient outreach via phone, text, or mail to remind and assist patients with completing overdue preventive screenings, labs, and follow-up appointments.
  • Collaborate with providers, care teams, and clinic staff to close care gaps and ensure accurate documentation in the EHR.
  • Monitor and track progress on organizational QI goals.
  • Analyze data trends and identify areas for targeted interventions.
  • Assist in developing workflows and staff education to support QI initiatives and compliance with payer and regulatory programs (e.g., HEDIS, CMS).
  • Maintain documentation for outreach activities and performance metrics.
  • Participate in QI meetings and support staff in implementing process improvements.

Requirements

Qualifications:

  • Education: High school diploma or GED (required) 
  • Bachelor’s degree in healthcare administration, public health, or a related field. (preferred)
  • Technical Skills: Proficiency in Excel and electronic health record (EHR) systems. 
  • Analytical Skills: Strong analytical and problem-solving skills, with the ability to interpret data.
  • Attention to Detail: High level of accuracy and attention to detail in data managing. 
  • Teamwork: Ability to work collaboratively with cross-functional teams and build strong working relationships.

Working Conditions:

  • Environment: Office-based role.
  • Travel: Minimal travel may be required for meetings.
