Position Title

Quality Improvement (QI) Coordinator

Reports To

Clinical Operations Manager

Position Summary

The Quality Improvement Coordinator is responsible for coordinating, monitoring, and advancing Open Cities Health Center’s quality, patient safety, and regulatory compliance activities. This role ensures alignment with HRSA, FTCA, ECRI, and internal QI/QA expectations through structured audits, proactive risk assessments, incident trending, CAPA management, and performance improvement initiatives across all departments.

Essential Duties & Responsibilities

Quality & Performance Improvement

  • Coordinate monitoring and improvement of UDS and internal quality measures
  • Track monthly performance trends and identify gaps requiring intervention
  • Support clinical teams with data-driven improvement strategies
  • Ensure inclusion of medical, dental, behavioral health, and optometry workflows in applicable measures

CAPA & Incident Management

  • Maintain a centralized Corrective and Preventive Action (CAPA) tracking system
  • Monitor CAPA ownership, timelines, evidence, and sustainment
  • Escalate overdue or high-risk CAPAs to leadership
  • Support incident trend analysis and committee reporting

Proactive Risk Assessments (FTCA / ECRI)

  • Coordinate required proactive clinical risk assessments
  • Support process mapping, failure mode identification, and mitigation planning
  • Track assessment findings through implementation and verification

Audit & Compliance Oversight

  • Conduct routine internal audits (vaccines, patient identification, documentation)
  • Validate accuracy of denominators and data sources (eBO vs internal data)
  • Support Annual Risk Management and FTCA documentation

Reporting & Committee Support

  • Prepare monthly and quarterly reports for QI/QC, Safety, Compliance, and Board committees
  • Maintain dashboards reflecting trends, risk scores, and improvement outcomes

Required Qualifications

  • Associates Degree in healthcare, public health, nursing, or related field (required); Bachelors Degree preferred
  • Experience in quality improvement, risk management, or healthcare compliance (preferred)
  • Familiarity with FQHC, HRSA, UDS, FTCA, or ECRI standards (preferred)
  • Strong data analysis, organization, and communication skills
