Process Improvement/Quality Improvement

Responsibilities:



Quality Improvement
·        Assist in the development, implementation, and evaluation of quality improvement initiatives

·        Collect, analyze, and interpret data related to performance metrics, patient outcomes, and process effectiveness

·        Prepare reports, dashboards, and presentations for internal and external stakeholders

·        Monitor compliance with internal standards, policies, and external accreditation/regulatory requirements

·        Facilitate QI activities and support quality-related training and education

·        Provide support and back-up to Accreditation Leads with coordination, documentation and readiness activities


Risk Management
·        Participate in incident reporting, investigation, and root cause analysis

·        Maintain accurate and confidential risk management documentation and logs

·        Monitor and trend incident data to identify areas of concern or patterns

·        Support risk assessments and help develop mitigation plans in collaboration with clinical and operational employees

·        Assist in preparing reports for internal committees and external regulatory bodies


Collaboration and Support
·        Work closely with clinical and administrative, and support teams to promote a culture of safety and continuous improvement

·        Support the Lead in audits, surveys, and preparation for accreditation reviews

·        Attend relevant committee meetings as assigned by the Lead

·        Provide support for quality or risk initiatives


Client Safety

Adhere to CMHA Durham client safety programs including program policies as well as infection control and prevention procedures. In partnership with clients, family members, and fellow staff, maintain open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Report client safety incidents and suggested improvements to manager or delegate.


Risk Management

Minimizes risks adhering to CMHA Policies and Procedures identifying, assessing, evaluating and controlling risks ensuring well-being of staff and clients 


Other Duties as Assigned

  • Attend, participate in team meetings and all employee meetings
  • Participate in organizational quality improvement activities, e.g. accreditation projects, committee work, special events
  • Participate in quality improvement activities at the team/program level
  • Participate in external committees as required

Skills:



  • Post-secondary degree preferred in Degree in Healthcare Administration, Nursing, Public Health
  • Certification in Lean Six Sigma, Quality Management, Healthcare quality, Healthcare Risk Management, Patient Safety and/or Canadian Risk Management
  • Knowledge and understanding of quality improvement and risk management principles with a minimum of 2 years’ experience in healthcare quality, patient safety, or risk management
  • Familiarity with Accreditation standards and process
  • Proficiency with data tools and QI methodologies
  • Excellent interpersonal and organizational skills; able to prioritize
  • Excellent oral and written communication skills
  • Excellent problem-solving and decision-making skills
  • Ability to maintain confidentiality
  • Strong knowledge in various aspects of applicable legislation
  • Effective client relationship building skills; understanding the client’s needs and background
  • Strong computer skills including PowerPoint, Excel (Macro’s an asset), Word, Visio and, Resolver, EMHware, Datix, (an asset)
  • Previous experience in working in Resolver, EMHware, Datix, experience an asset
  • Ability to work independently and co-operatively as part of a team, management, board of directors, stakeholders and partners such as external vendors
  • Able to integrate CMHA Durham’s philosophy of being a “welcoming organization” into everyday practice.
  • General knowledge of the mental health environment in Ontario.
  • Ability to adapt to change and navigate through ambiguous situations
  • Project management skills
  • Fluency in French considered an asset.
  • Work outside of normal business hours as required
