Description

Primary Duties and Responsibilities

The Pulmonary Disease Patient Navigator (PDPN) identifies all potential patients for participation in a pulmonary disease management program using evidence-based metrics and clinical protocols. The PDPN provides real-time clinical assessment of each patient to determine entrance criteria into the program. The PDPN coordinates all aspects of the patient’s care continuum to include, but not limited to:

  • clinical evaluation
  • education
  • fiscal resourcing
  • discharge planning
  • insurance verification support
  • after-care follow-up
  • home health agencies/durable medical equipment (DME) providers
  • direct interaction with primary care and/or specialists

The PDPN will be responsible for essential data collection and interpretation, outcome analysis, and routine presentations on the progress of the Disease Management Program to the appropriate staff/leadership team(s). The PDPN will work in concert, collaboratively in a multi-disciplinary approach to coordinate all aspects of care and follow up necessary for the successful management of the patient’s disease process as well as exploring opportunities to proactively guide wellness.

The PDPN is responsible for Pulmonary Disease Management Program promotion and expansion of the program to the medical staff, community and beyond via educational in-services, health fairs, and other promotional venues. The PDPN ensures compliance with regulatory agencies with billing and documentation practices. The PDPN will provide guidance to both patient and colleagues by conducting team conferences to develop and implement a detailed care plan for each patient as well as progress and follow up plans as appropriate. The PDPN projects an image of professionalism in communication, appearance and conduct. The PDPN will actively participate in the continuous quality improvement process while maintaining a working knowledge of departmental and hospital standard operating procedures. The position may require travel between Hoag Hospital (Hoag) and its affiliates. Performs other duties as assigned.



Qualifications

Education and Experience

  • Associate's degree in Respiratory Therapy from an accredited Respiratory Care program required.
  • Bachelor’s degree in Respiratory Care, Health Services Management, or related studies preferred.
  • Minimum five (5) years of experience as an RCP providing respiratory care interventions to patients, with minimum of two (2) years in a leadership role within the scope of Respiratory Care
  • Prior or current experience as a Pulmonary Rehabilitation Coordinator and/or Supervisor, Patient Case Manager, Home Health/DME Leadership Role, working with Geriatric population health, specifically respiratory care preferred

License Required

  • Current license to practice respiratory care issued by the Respiratory Care Board of California (RCP)
  • Registered Respiratory Therapist (RRT) credential verified and granted by the National Board of Respiratory Care (NBRC), must remain current for the entire term of employment

  • Current Basic Life Support (BLS) Certification granted by the American Heart Association
  • Advanced Cardiac Life Support (ACLS)

Certifications Preferred

  • Adult Critical Care Specialist (ACCS)
  • Neonatal Pediatric Specialist (NPS)
  • Asthma Educator (A-Ed) Certified
  • Registered Pulmonary Function Testing (CPFT or RPFT)
