Summary

Collaborate with team members to improve health outcomes for high-risk populations.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Cross reference clinical databases to identify high-risk populations
  • Participate in quality data collection for programs focused on improving health outcomes
  • Communicate with clinical team regarding high-risk population needs and engagement
  • Facilitate connecting high-risk populations with appropriate community resources.
  • Document interactions and collaborative efforts in electronic medical record
  • Help with needed wellness services for clients when requested
  • Assist with staff wellness when requested
  • Attend required clinical meetings prepared with information to advocate and benefit high-risk populations
  • Attend staff meetings, in-service training, workshops, and seminars and adhere to agency policy, rules, regulations, and procedures
  • Performs other duties as required

QUALIFICATIONS

  • High School Diploma or General Equivalency Diploma required
  • Bachelor’s Degree in mental health or health-related field preferred.
  • Minimum two years relevant experience required
  • Oklahoma Department of Mental Health and Substance Abuse Certified Case Manager I, if eligible, and Wellness Coach required within 90 days of hire required (training provided after hire)
  • Peer Recovery Support Specialist certification, if applicable, preferred
  • Valid OK driver’s license, clean driving record, reliable transportation required
  • Excellent verbal, written and electronic communication skills required

PHYSICAL REQUIREMENTS

  • Must have the ability to stand or sit for long periods of time
  • Must have the ability to lift, push, or pull a minimum of 25 pounds
  • Ability to travel approximately 10% of the time
  • Ability to use telephone, PC, fax machine, copy machine, and printer
