POSITION SUMMARY:

The Patient Navigator - Young Adult Care and Prevention (YACAP) is an entry-level role that supports youth and young adults ages 13–29 in navigating health care and prevention services, with a focus on HIV prevention and PrEP enrollment. The Patient Navigator works closely with the Case Manager and Program Manager to assist participants with appointments, insurance and program enrollment, recruitment and outreach, and practical problem-solving so participants can access and stay engaged in care.


DUTIES AND RESPONSIBILITIES:

    1. Assesses needs of patients seeking and accessing PEP and PrEP related services.
    2. Assist providers with identifying patients for PEP/PrEP and assist with intakes/program enrollment, service navigation, education and follow up around PEP and PrEP services
    3. Assists with data collection and data entry for the AIDS Institute Reporting System and CHN’s Electronic Medical Record
    4. Coordinates medical services with clinic staff.
    5. Access to PHI during work activities and adherence to all HIPAA, corporate compliance, and Health Home guidelines.
    6. Provide a wide range of community education and increase awareness about Treatment as Prevention for HIV positive individuals and Pre-Exposure Prophylaxis (PrEP) Post-Exposure Prophylaxis (PEP) for those at high risk for HIV infection.
    7. Participate in health fairs, tabling, and joint health activities with other community agencies.
    8. Participate in training and support services.
    9. Maintains strict confidentiality of all patients related information.
    10. Facilitates PEP/PrEP related services for health center patients as appropriate with respect to their confidentiality and privacy.
    11. Increase the number of undiagnosed people who know their HIV status and are linked to care.
    12. Identify, link, and retain people diagnosed with HIV and ensure prompt access to medical care, partner, and prevention services.
    13. Navigate patients into integrated STI and HVC education and linkage to STI and HVC screening and treatment.
    14. Special projects as assigned by the Program Manager or other ID/LGBTQ leadership team members. 
    15. Participates in designated program meetings.
    16. Participates in Center staff meetings.
    17. Participate in relevant internal and external training.

EDUCATION & EXPERIENCE:

  • High School Diploma or satisfactory equivalent preferred.
  • Min of one year of experience in community work and patient navigation/linkage preferred.
