Job Summary
The Patient Navigator supports colorectal cancer screening efforts within a Virginia Federally Qualified Health Center (FQHC). This role facilitates patient care coordination, assists patients in overcoming barriers to care, and maintains accurate tracking and case management documentation. The Patient Navigator must demonstrate strong listening skills, empathy, cultural awareness, willingness to learn, and the ability to maintain confidentiality at all times.
Essential Duties and Responsibilities

- Communicate with patients, families, and caregivers using culturally and linguistically appropriate methods.
- Facilitate communication among patients, providers, care teams, and partner clinics.
- Assist patients in navigating the healthcare system, including scheduling appointments, coordinating transportation, obtaining referrals, accessing childcare, and securing interpreter services.
- Educate patients and families on recommended screenings and treatments, adjusting communication to different levels of health literacy.
- Provide reminders, follow-ups, and ongoing support during the colorectal screening and follow-up process.
- Assist patients in preparing for diagnostic tests and understanding care instructions.
- Conduct barrier assessments and develop individualized care plans.
- Advocate for patients to ensure timely access to required services and follow-up care.
- Coordinate specialty appointments as needed.
- Document all patient interactions, navigation activities, and care plans in the electronic health record (EHR).
- Maintain patient navigation tracking tools and prepare monthly reports.
- Follow all HIPAA requirements and confidentiality standards.
- Establish community–clinical linkages and develop referral partnerships.
- Collaborate with internal champions on cancer screening initiatives.
- Support a team-based approach to patient care and workflow.
Schedule
- Monday, Tuesday, Thursday and Friday: 8:00 AM–5:00 PM
- Wednesday: 8:00 AM–11:00 AM
- Weekend work and additional hours may be required.
- This is an on‑site position.

Education and Experience Requirements
- Bachelor’s degree or equivalent combination of education and experience.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred.
- Minimum of four (4) years of related experience and/or oncology training preferred.
- Minimum of four (4) years of clinical experience; case management experience preferred.
Knowledge, Skills, and Abilities
- Experience in medical, public health, or community health settings.
- Experience working with diverse populations and community partners.
- Strong organizational, interpersonal, and communication skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Proficiency in Microsoft Office (Word, Excel).
- Experience using electronic health record (EHR) systems; eClinicalWorks preferred.
