Patient Navigator (Marketer) – Hospice

Advanced Home Health and Hospice of Cleveland | Field-Based | Full-Time

Are you an outgoing, motivated professional with a passion for connecting people to care? Advanced Home Health and Hospice of Cleveland is looking for a dynamic Patient Navigator to join our growing team. This is a field-based role focused on building strong relationships and promoting our exceptional hospice services in the community.


About the Role

As a Patient Navigator, you'll spend the majority of your time out in the field, meeting with providers, educating community partners, and establishing referral relationships. This is not a desk jobβ€”we're looking for someone who thrives on meaningful interactions, strategic outreach, and making connections that help patients receive the care they need.


What You'll Do

  • Promote hospice services to physicians, hospitals, discharge planners, and community groups
  • Conduct regular outreach and in-person visits to maintain and grow referral networks
  • Educate providers and partners on hospice care and eligibility
  • Represent Advanced Hospice of Ohio at community events, health fairs, and networking opportunities
  • Track leads, referrals, and activity in the CRM system
  • Collaborate with clinical teams to align messaging and ensure quality patient transitions
Requirements
  • Previous marketing experience (required)
  • 1+ year of healthcare marketing or field outreach experience (preferred)
  • Knowledge of hospice or healthcare services is a plus
  • Valid driver’s license and reliable transportation (required)
  • Strong communication and presentation skills
  • Self-driven and organized, with a passion for meaningful work
