Pay Rate-$24.15-$32.68 (Compensation will be determined based on qualifications and experience)

About the Role:

Health Equity Patient Navigators (PNs) help HICHC patients address social and health-related needs. They support patient wellness and independence through assessment, education, service coordination, advocacy, and addressing non-clinical factors that affect health, working in both clinical and community settings.

Minimum Qualifications:

  • Required-High School graduate or GED certificate.
  • Required-Two years related experience in health or social services field
  • Preferred-Bachelor’s degree in Human Services field strongly preferred
  • Preferred-Strong understanding of local community resources and social services agencies

Responsibilities:

  • Partners with the patient to address the non-clinical barriers that patients face, with a focus on the social factors that negatively affect the patient’s health (i.e., food security). 
  • Provides care coordination and case management to target the health needs identified by the patient and the care team.
  • Provides brief counseling (i.e., health education, motivational interviewing, behavioral activation, nicotine cessation, and problem-solving treatment) to support patients in reaching health goals.

The role requires compassion and professionalism, the ability to work independently and as part of a team, strong problem-solving and prioritization skills, technological proficiency, and the capacity to travel to clinic and community locations with a valid driver’s license and insurance.
