Position Description:
 

To help patients access the full continuum of cancer care from screening to detection, diagnosis, treatment, and resources, available beyond the completion of treatment. Concurrently facilitates proactive patient care services to ensure that care is appropriate, timely, cost effective and achieves the desired outcome; including, but not limited to, psychosocial assessment, patient advocacy, discharge planning, resource management, crisis intervention, community education, and counseling, designed to overcome barriers to care and meet the varying psychosocial needs of patients and their families relating to a cancer diagnosis.

Qualifications:
1. Associate's or Bachelor's Degree in a healthcare-related field (e.g., Nursing, Allied Health, Social Work, or Public Health) required.
2. Clinical experience in healthcare or social services settings is preferred.
3. Strong communication, assessment, and care coordination skills; knowledge of healthcare systems and resources.
