The Patient Navigator will engage patients, create a trusting relationship, navigate patients through completing the Colorectal Cancer Screening process (including, but not limited to FIT testing) and closely communicate with a patient’s care team based on clearly outlined goals and identified patient needs. The Patient Navigator will aid patients in the coordination and completion of appointments related to Colorectal Cancer Screening test results and help decrease barriers to timely care and treatment.  The Patient Navigator will participate as a contributing member to the HRSA Accelerating Cancer Screening Grant.

Roanoke Chowan Community Health Center will provide open access, community-oriented primary care of the highest quality to all residents, migrants and seasonal farmworkers, as well as, visitors of Hertford, Bertie, Northampton, Gates, Washington and surrounding counties, regardless of their ability to pay.

Ability to gather and synthesize information. Must be void of any drug convictions or related charges. Able to travel to other RCCHC site. Education: Bachelor’s Degree from a state-accredited school in Public Health or Social Work or at least 3-5 years’ experience in case management, social work or public health. 

Experience: 

• Experience working in a multi-cultural setting
• Minimum 2 years in a health care or public service office setting
• Experience in patient case management and coordinating community resources
• Basic computer skills required, electronic medical record (EMR) experience preferred

Certification(s)/Licensure: N/A

Knowledge:

• Knowledge and understanding of Federally Qualified Health Centers (FQHC’s) in regard to the Federal 330 Public Health Service Act
• Knowledge of computer systems and applications.

• Assist in completing activities around patient mailings including generating and quality checking mailed packets, receiving and responding to incoming participant telephone calls, and conducting patient reminder calls.
• Responsible for contacting patients over the phone to assist in setting up colonoscopy appointments and provide reminder phone calls prior to every appointment. 
• Provide navigation for patient by addressing any barriers, including transportation, financial, literacy and language
• Document and track navigation activities, provide regular reports to the grant team on cases, and maintain summary data on case statuses.
• Maintains patient database and protects confidentiality of patient information and records.
• Maintain compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation.
• Performs other duties as assigned.
