Remote Patient Monitoring/Remote Therapeutic Monitoring
Description

The Medical Assistant (MA) supports clinical operations by assisting with Remote
Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. This role
involves patient outreach, data collection, care coordination, and ensuring patients
remain engaged with their care plans. The MA works closely with providers, nurses, and
care coordinators to improve patient outcomes, especially for individuals with chronic
conditions.

Key Responsibilities
 Monitor and review patient-generated health data (e.g., blood pressure, glucose
levels, weight) through RPM platforms
 Conduct routine patient outreach via phone or digital communication to support
CCM and RPM services
 Assist in onboarding patients into RPM and CCM programs, including device
setup and education
 Document patient interactions, vital signs, and care coordination activities in the
electronic health record (EHR)
 Identify abnormal readings or patient concerns and escalate to clinical staff as
appropriate
 Reinforce care plans and educate patients on medication adherence, lifestyle
modifications, and disease management
 Schedule follow-up appointments and coordinate care between providers and
patients
 Ensure compliance with CMS guidelines for RPM and CCM services, including
accurate time tracking and documentation
 Maintain confidentiality and adhere to HIPAA regulations



Qualifications

 High school diploma or equivalent (required); completion of an accredited
Medical Assistant program (preferred)
 Current Medical Assistant certification (e.g., CMA, RMA) preferred
 Experience in primary care, chronic disease management, or telehealth preferred
 Familiarity with EHR systems and remote monitoring technology
 Strong communication and patient engagement skills
 Ability to work independently and manage multiple tasks
 Basic understanding of chronic conditions such as hypertension, diabetes, and
heart disease
