PRIMARY RESPONSIBILITIES:

This individual is responsible for educating, training and supporting the clinical team using population health strategies.

While focused on the daily tasks and flow of the clinic, this role will identify process improvement activities, create

workflows, implement change and monitor progress to maximize patient care. The ability to quickly adapt and address

clinical needs of the patient and support staff are essential. The MCS works closely with the DON to support the 

delivery of safe, high-quality patient care across all nursing services. In addition, this individual will collaborate with the DON to align clinical support operations with nursing leadership priorities, organizational goals and regulatory requirements.  

  1. Manage healthcare teams 

      a. Direct management of a clinical team encompassing RN’s, PCC’s and MA’s.

      b. Able to define problems, formulate solutions, make recommendations, and initiate corrective actions leading to positive outcomes.

      c. Perform patient care duties when needed as well as respond to medical emergencies.

      d. Strong leadership and coaching skills to develop cohesive and successful team members.

      e. Effectively use data to analyze performance, set program goals, implement processes, and achieve team and quality initiatives.

      f. Demonstrate the ability to meet and exceed established targets and deadlines.

      g. Excellent interpersonal and communication skills to build and maintain collaborative relationships.

      h. Highly organized, able to perform a variety of tasks independently, timely and effectively.

2. Collaborate as an integral part of a multi-disciplinary team providing health care

     a. Collaborate with the HCA’s to achieve mutual goals with respect to the clinical teams’ day-to-day operations with a focus on efficiency, service, and          

           teamwork.

     b. Assist with relationships with key partners; Missouri Primary Care Association (MPCA), Missouri Family Health Council (MFHC), MO Dept of Health   

          and Senior Services (HSS), Health Choice Network (HCN), and other agencies and community partners.

      c. Support organizational efforts for improving and achieving metrics established by FCHC, Health Resources & Services Administration (HRSA), 

           Primary Care Health Home (PCHH), Accountable Care Organization (ACO), Patient Centered Medical Home (PCMH) and others as identified in 

           partnership or regulatory compliance.

       d. Ensure regulatory compliance and enforce clinical and quality standards with attention to supplies, O2 delivery, sterilization/disinfection processes,

            equipment maintenance, and certification and licenses.

        e. Attend and participate in departmental and organizational meetings.

3. Supervise RN, PCC and MA teams to ensure success and drive consistent and quality patient care

        a. Identify opportunities for improved clinic, team and patient workflows. Working with the Director of Nursing (DON), outlines plans, implements 

             changes, and monitors effectiveness through ongoing evaluation and feedback.

        b. Provide direct oversight for all patient care tasks, clinic duties and team responsibilities.

        c. Ensure staff are delivering quality, evidence-based care and adhering to best practices.

        d. Perform routine education and competencies for staff, including medication and injection administration, dressing changes, vital sign assessment, 

             point of care testing (POCT), EKG, nebulizer treatments, oxygen administration, nasopharyngeal/oral swabs, patient teaching and other 

             necessary RN, PCC and MA duties.

         e. Perform routine OB-specific competencies for staff, including OB and WH procedures.

         f. Ensure a high-quality patient rooming process including thorough documentation of the chief complaint, vital signs, medication review, health 

             screenings, care gaps, patient teaching, provider communication and appointment scheduling.

          g. EHR related tasks, document workflows and identify areas for improved efficiencies in the in-basket, focused on triage, patient calls, My Chart 

               messages, RX refills, transitions of care and medication refills.

          h. Review and audit charts for compliance, identify gaps, and collaborate with clinical staff to provide feedback and ensure gap resolution.

          i. Along with the DON, analyze and monitor operational and clinical data to ensure compliance with established benchmarks and performance 

              standards, then direct team on areas to perform more efficiently and effectively.           

          j. Effectively use quality tracking tools, DRVS, power BI, population health platforms, EPIC EHR reports and dashboards to manage all populations.   

          k. Facilitate and lead team meetings and pre-visit planning with care team members. 

          l. Responsible for payroll, schedule, PTO, performance reviews and personnel issues.

         m. Perform all other duties as assigned.

Requirements

QUALIFICATIONS :

a. Graduate of an accredited school of nursing required; master’s degree in nursing (MSN) preferred. 

b. A valid, active Registered Nurse license in the state of Missouri is required, and in good standing.

c. Minimum of 5 years nursing experience working with patients who have complex conditions requiring care coordination and collaboration with a multidisciplinary care team.

d. Minimum of 2 years in a management or leadership role, in a medical setting, with direct reports.

e. Experience working with patients in a community health care setting.
