The Integrated Behavioral Health Clinical Liaison delivers clinical services directly to patients within an integrated team-based care model. These services may include conducting psychosocial assessments and consultations (either urgent or pre-scheduled), providing support and linkage services, delivering psychoeducation, offering brief interventions, and providing counseling aimed at improving the patient's physical and psychological well-being. The IBH Liaison will manage the coordination of medical and behavioral health services for patients served by BVCHC and Certified Community Behavioral Health Clinics (CCBHC) to ensure a comprehensive, holistic approach in delivering Home-base services to all eligible patients. Home-based care refers to person-centered systems that enable easy access to and integration of a complete range of primary care, acute behavioral health services, long-term community-based services, and community supports. The IBH Liaison, as part of the Integrative Behavioral team, provides clinical supervision to behavioral health interns, participates in quality improvement and peer reviews, and assists in developing programming as requested by the Director of Behavioral Health Services.

EDUCATION, EXPERIENCE, & SKILLS

  • A valid clinical license to practice in Rhode Island as an LCSW or LMHC is required; LICSW qualifications are preferred.
  • A Master's Degree, including appropriate practicum, internship, and postgraduate work focused on clinical mental health practice, is required.
  • At least two years of experience in providing direct clinical behavioral health services is preferred.
  • Proficiency in designing and implementing clinical pathways and treatment protocols for specified chronic conditions is essential.
  • Experience treating substance abuse/addictions is preferred.
  • Experience working in a community health care setting is preferred
  • A thorough understanding of behavioral medicine and evidence-based treatment for both medical and mental health conditions is essential.
  • Proficiency in managing the dynamics of primary care, collaborating with an interdisciplinary team, and possessing excellent communication skills are essential.
  • Excellent patient engagement skills are required.

OTHER REQUIREMENTS

  • Reliable transportation is required
  • Proficiency in both English and Spanish, Cape Verdean Creole, or Portuguese is preferred.
  • Cultural sensitivity is essential when working with a diverse patient and staff population.
  • The role requires both independent and collaborative work skills.
  • Knowledge of computers and medical records is required
  • Knowledge of Microsoft Suite is preferred
