SUMMARY OF JOB RESPONSIBILITIES: The Social Work Supervisor provides leadership, coordination, and oversight for hospice and palliative care social work services. This role ensures the delivery of high-quality, evidence-based, trauma-informed psychosocial care across HopeWest.
Key responsibility of the supervisor is to serves as a clinical resource and promotes excellence in delivery of patient care, documentation, development of standards of care, and interdisciplinary collaboration. Additionally, the Social Work Supervisor assists in the oversight of time-off requests and scheduling for Social Workers, coverage coordination, supervision of variable staff, compliance monitoring, and facilitation of discipline-specific meetings.
QUALIFICATIONS
- Must be a Licensed Clinical Social Worker (LCSW) with active registration through Colorado DORA.
- 2 years of LCSW experience preferred.
- Advanced Certification in Hospice and Palliative Care preferred.
- Must be able to provide clinical supervision for LCSW candidates.
- Advanced training in trauma-informed care, EMDR, DBT, and crisis intervention preferred.
- Valid Colorado driver’s license, adequate automobile insurance, and unlimited access to reliable transportation maintained throughout employment.
COMPETENCIES & REQUIREMENTS:
- Computer proficiency in Microsoft products (Word, Excel, and Outlook) and EMR.
- Ability to cope with patient/family emotional stress within the framework of the hospice philosophy.
- Participates in the on-call, holiday, weekend, and after-hour’s rotation.
- Strong ability to develop policies, lead staff training, and provide clinical supervision.
- Excellent interpersonal, problem-solving, and leadership skills.
- Ability to work collaboratively within an interdisciplinary team and adapt to evolving patient needs.
- Ability to interact appropriately with geriatric populations and clients with cognitive behavior impairment and other various barriers to communication.
- Demonstrates a driven curiosity for knowledge of available community resources for patients/families.
- Current CPR certification is maintained throughout employment.
