The Clinical Educator at Transitions Care serves as a key member of the corporate clinical team and is responsible for the development, implementation, and oversight of standardized clinical education and competency programs across a multistate hospice organization. Reporting to the corporate clinical leadership team, this role ensures consistent, compliant, and high-quality clinical practice by supporting onboarding, ongoing education, regulatory readiness, and professional development for interdisciplinary team members.


Major Responsibilities

Education

The Clinical Educator is responsible for assisting in the training and onboarding of new team members when locations have a qualified need.

  • Act as a mentor who facilitates individual staff development from novice to expert.
  • Participate in the orientation of new personnel and assists with field training as needed.
  • Teach patient/family/caregiver disease process and self-care techniques as appropriate. Provide medication, diet and other instructions as ordered by physician and recognize and utilize opportunities for health counseling with patients/families/caregivers.
  • Facilitate the development of teaching aids to promote patient/family/caregiver learning.
  • Evaluate effectiveness of educational experiences in order to promote ongoing professional development of hospice staff and quality patient/family/caregiver outcomes.

Leadership

The Clinical Educator represents Transitions across multiple agencies throughout the state, and should be seen as a resource to all staff and team members.

  • Demonstrates behavior appropriate to role model and leader. Adherence to a code of conduct conducive with Transitions HealthCare policy is expected.
  • Demonstrates creativity and critical thinking ability in the development of patient care plans
  • Functions without supervision, utilizing time constructively and organizing assignments for maximum productivity.
  • Collaborates with the Interdisciplinary Team and Executive Director and Administrator to develop agency goals and objectives.

Other

  • Field Support in time of staffing crisis as determined by the Chief Nursing Officer
  • Adherence to a code of conduct conducive with Transitions Hospice policy is expected.
  • Meet or exceed delivery of Company Service Standards in a consistent fashion.
  • Interact with all staff in a positive and motivational fashion supporting the Company’s mission.
  • Conduct all business activities in a professional and ethical manner.
  • Create and maintain a professional atmosphere that is also a fun place to work.
  • Travel required >70%

Requirements

Education and Experience

  • Bachelor’s degree in Nursing (BSN) preferred.
  • RN licensure in the primary state of assignment, with ability to obtain in other states. Compact licensure is acceptable, if applicable.
  • C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred, but not required
  • Five (5) + years general nursing preferred. Experience with pain and symptom management interventions preferred.
  • Minimum 2 years of hospice experience
  • Strong ability to teach, train and educate others
  • Ability to work within an interdisciplinary setting.
  • Demonstrate excellent observation, assessment, nursing judgment and communication skills.
  • Strong ability to multitask, and prioritize patient needs.
  • Valid driver's license and reliable transportation.
  • Must be available to travel >75%

$90,000 - $95,000 a year
