JOB DESCRIPTION
• Provide private care for the elderly in their homes and facilities.
• Follows care guidelines outlined in the client's individualized care plan.
• Performs but is not limited to: personal care assistance, Alzheimer's/Dementia Care, stroke recovery, ambulation/mobility assistance, transportation needs, meal preparation, and housekeeping.
• Reports any observed or reported changes in the client's condition and/or needs to the office in a timely manner.
• Documents care provided and completes the forms required for the client's records.
• Communicates effectively with all members of the office staff.
• Maintains confidentiality in all aspects of the job.
• Willing to travel up to 30 mile radius of our Lawernceville office to serve clients.
