Job Description
Develops numerous strategies for education and facilitates health curriculum for patients living with HIV/AIDS. Connects patients with resources to overcome food insecurity challenges, and other Social Determinants of Health.
Provides direct case management and care coordination services to people living with HIV who are out of care (OOC), newly identified in care (NIC), or at risk of falling out of care (RFC). Conducts comprehensive intake assessments, develops individualized care plans, and provides ongoing care coordination to support sustained engagement in HIV care and viral suppression. Work in collaboration with the multi-disciplinary team (Medical Provider, Nurse, Social Worker, and Health Education Team). Facilitates linkage to internal and external supportive services and provides health education to enrolled clients. Provides Risk-Reduction counseling, works directly with Sexual Health.
