Job Summary:  

The Health Educator plays a vital role in implementing, coordinating, and facilitating group education and outreach programs for the Healthy Start Program. This position leads group healthcare and education initiatives, works collaboratively with the Women’s Health Department to implement Centering Pregnancy, and assists in identifying and recruiting eligible prenatal patients. The Health Educator also manages documentation, tracking, and reporting of all group education activities while ensuring compliance with Hamilton’s quality and regulatory standards.

Essential Duties and Responsibilities:

  • Serve as lead on implementation of group education.

  • Guide and lead the development of group education and group healthcare initiatives.

  • Work closely with Hamilton’s Women’s Health Department to implement Centering Pregnancy and identify and recruit eligible prenatal patients.

  • Facilitate all group education sessions both at Hamilton and in the community.

  • Co-facilitate Centering Pregnancy groups at Hamilton Health Center sites.

  • Track and document all group education initiatives.

  • Conduct outreach and recruitment for group education activities.

  • Attend and assist in required classes, meetings, in-service trainings, and community activities.

  • Maintain confidentiality and security of information when providing details to internal and external sources.

  • Develop reports and deliver presentations to internal and external stakeholders as required.

  • Work within the parameters defined by Hamilton’s Quality Department and follow established protocols to meet the requirements of regulatory agencies.

  • Participate in Hamilton’s efforts related to the revenue cycle, including productivity, meeting or exceeding revenue targets, and managing expenses as budgeted.

Minimum Education/Certifications: 


Bachelor’s degree in Human Services, Social Work, Public Health, or related field.

 

Minimum Work Experience:  


Minimum of two years’ experience in Human Services or working with children and families in Public Health, Outreach, or Social Work preferred.

Knowledge of The Joint Commission regulations, prior CPR/First Aid certification, and bilingual speaking skills preferred.

Reliable transportation to travel throughout the service area.
