About the Role
As a Care Manager, you’ll provide telephonic education and coaching to patients managing chronic conditions such as diabetes, hypertension, and obesity. You’ll be part of a supportive program designed to improve patient outcomes and lower healthcare costs — and you’ll be trained in health coaching methods to help patients make positive lifestyle changes.
Key Responsibilities
- Identify and engage patients who could benefit from coaching.
- Conduct telephonic outreach to explain the program and encourage enrollment.
- Educate and motivate patients toward healthier behaviors by using motivational interviewing and behavior change science.
- Document interactions clearly and accurately in our system.
- Triage and refer patients to appropriate services as needed.
What We’re Looking For
Education & Experience
- Previous experience in an employee wellness or health promotion role preferred.
- Degree in health-related field required (such as nutrition, public health, exercise physiology,, health science or health education).
- Professional certifications in wellness, health promotion, or related fields preferred.
- Minimum 2 years of experience coaching members one-on-one with health goals.
- Spanish-speaking ability is a plus.
- Minimum internet speed requirement: 25 Mbps download / 25 Mbps upload.
Skills & Abilities
- Strong communication, listening, and motivational interviewing skills.
- Ability to follow detailed procedures and work independently.
- Effective decision-making and problem-solving abilities.
- Proficiency with Microsoft Outlook and Windows-based software.
- Strong multitasking and organizational skills.
