Forum Health is a nationwide network of medical providers and practices specializing in functional medicine. We are looking for a functional medicine Health Coach to join our team and work directly with clients in their health transformation journey.
The role would be M-Th, 8:30 am CT to 6 pm CT and Friday’s 8:30 am CT to 2 pm CT. Work hours could change depending on which time zone you reside in.
The Health Coach is a key role of Forum Health and is responsible for assisting and educating patients about nutrition, lifestyle changes, and personalized support to address the root causes of health challenges and enhance overall well-being. As a Health Coach, you will leverage your expertise to guide both current and new patients in taking control of their healthcare, offering them a supportive partnership on their wellness journey.
Duties and responsibilities include the following.
- Build coach-provider relationships.
- Follow-up patient communication with summary and recommendations after each client session
- Chart patient encounter in EMR and close chart note within 24 hours of visit
- Educate clients about their health/chronic illnesses in a non-academic manner.
- Educate patients on Forum Programs and provider-specific dietary recommendations.
- Manage client community pages, addressing general inquiries, providing support and educational material
- Diligently track your clients’ progress. Provide ongoing feedback, advice, and encouragement.
- Constantly problem-solve with your clients if things seem to not be working.
- Communicate with your clients through video calls and electronic messaging.
- Ensures patient Electronic Medical Records (EMR) and correspondence are accurate and up-to-date in the EMR system and makes updates as needed and appropriate.
- Provides support to providers and assists other medical staff as needed.
- Provides supplement and nutraceutical information to patients and answers questions as needed.
- Drive process improvement in the optimization of workflows or the development of new tools, training, or services to meet the needs of the patient experience.
- Respects patient confidentiality with a thorough understanding of the HIPAA/HITECH laws.
- Work in an evolving environment and be able to easily pivot as priorities shift.
- Travel as needed to meet business objectives (estimated travel up to 5%).
- Other duties may be assigned.
Requirements Education/Experience
- Bachelor’s Degree in Healthcare or applicable science.
- Must be NBHWC Certified, health coaching certification from an accredited health coaching school with a robust program (i.e. Institute for Functional Medicine, Nutritional Therapy Association); Registered Dietitian, CHES, ideally certified by the National Board of Health &Wellness Coaching.
- Certified Nutrition Specialist (CNS) certification highly preferred.
- In lieu of a degree, relevant experience, three years of health coaching experience, a strong understanding of the functional medicine view of health concerns and the ability to credibly advise patience
- A high level of ownership, is a good listener and persuasive, has strong written and verbal communication skills, ability to establish a relationship with a variety of individuals, and has exceptional organizational skills and follow-through on tasks.
- Candidate must be comfortable working in a fast-paced start-up environment, with the ability to quickly adapt and switch between multiple tech platforms to manage client interactions, data, and workflows efficiently.
- Candidate must be able to type 40 words per minute.
- Preference will go to candidates with over four years of experience coaching clients one on one to meet their health goals, experience with Electronic Medical Records or Electronic Health Records and medical office experience.
- Applicant must be authorized to work in the United States without the need for sponsorship now or in the future.
Salary: $22 - $24 per hour
