Health Coach/Community Support Worker
Location: South Otago
FTE: 0.6 (Tuesday, Wednesday & Friday)
We are seeking an experienced Health Coach who is passionate about supporting whaiora (a patient seeking health support) to manage their health and wellbeing. We are keen to hear from applicants interested in a career that supports people within their community who want to make lifestyle changes to better manage their health.
This diverse and fulfilling role empowers whaiora and whanau to gain the knowledge, skills, and self-confidence to reach their own health and wellbeing goals. This includes lifestyle changes (smoking cessation, exercise), promoting self-management (especially for managing long-term conditions like diabetes), facilitating groups and helping navigate primary care and the wider social services as needed for success.
About the role:
Health Coaches are a high-impact, accessible resource within general practice, supporting 8-10 whaiora a day. Empowering individuals, whanau and groups to set goals and make positive changes in areas such as:
To be successful in this role, you will:
You will be well-supported in this role with initial training in Health Literacy and a comprehensive 3-month induction to help integrate into practice. We value your growth and development as a Health Coach, providing additional professional development to support you in this role.
This role will be across three rural South Otago Practices.
