Te Korowai Hauora o Hauraki is a not-for-profit kaupapa Māori health organisation, based in Hauraki. Providing medical, wellness and community services in Coromandel, Paeroa, Waihi, Te Aroha and Whitianga areas, we have successfully delivered services in Hauraki since 1994. We aim to be a leading indigenous health provider by providing high-quality, whānau-centred services in the Hauraki locality.
We are looking for a Health Coach to join our Ohu Hinengaro (primary mental health) team. This full time (1.0 FTE) non-clinical role will be based at a designated general practice clinic in Hauraki.
Reporting to the Hinengaro Team Lead, the role will work closely with our Whānau Health Centre team members, general practice teams, Te Whatu Ora - Mental Health & Addiction colleagues and other Te Korowai Hauora o Hauraki services, to ensure whānau needs are met.
The Health Coach is actively engaged in utilising an integrated ‘Stepped Model of Care’ that supports whānau needs and enables them to move seamlessly between services. Our Ohu Hinengaro team also uses a Mahi a Atua approach to transforming systems, processes, and outcomes for whānau and is designed to address health inequalities for Māori, provide whānau with service choices and improve outcomes for whānau. The model is underpinned by Feedback Informed Treatment an evidence-based practice where Mataora/ practitioners gather real-time feedback from whānau using structured measures to identify what is and is not working and then adjust our practice to better meet all whānau needs.
