Position Summary

We are seeking a dependable and clinically skilled Licensed Vocational Nurse (LVN) to support our Remote Patient Monitoring (RPM) program. This role involves reviewing patient data, managing chronic conditions remotely, and collaborating closely with providers to ensure timely interventions.

Key Responsibilities

  • Monitor and review daily RPM alerts (blood pressure, glucose, weight, pulse ox, etc.)
  • Assess patient readings and identify clinical concerns
  • Contact patients regarding abnormal readings or non-compliance
  • Provide patient education on chronic disease management and device use
  • Triage and escalate abnormal findings to providers appropriately
  • Document all patient interactions and readings accurately in the EMR
  • Assist with onboarding and educating patients in the RPM program
  • Support care plan implementation and follow-up


Qualifications

Qualifications

  • Current LVN license in the state of Texas
  • Experience in primary care, chronic care management, or RPM preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient engagement abilities
  • Experience with EMR systems (eClinicalWorks preferred)
  • Ability to work independently and manage multiple patients daily

Bonus Skills (Preferred)

  • Experience with RPM or care management programs
  • Knowledge of hypertension, diabetes, and chronic disease protocols
  • Bilingual (Spanish) is a plus
