BASIC PURPOSE AND SCOPE:
Under the direction of the Chief Executive Officer, is responsible for, but not limited to, the direct supervision of all aspects of nursing for home care, hospice and palliative care operations and services. This includes ensuring the necessary planning, implementation, reporting, and ongoing oversight of clinical services, including nursing, social work and counseling, bereavement services, spiritual care, contracted therapies, scheduling of paraprofessional services, and coordinating use of patient care volunteers related to the delivery of home care and hospice services. This position will ensure appropriate interdisciplinary communications with all members of the clinical team, to pre-mentioned disciplines as well as the HC&H Medical Director, and IDT physicians. In collaboration with Quality/Compliance, will insure ongoing clinical policy development and annual review as required; maintains a focus on compliance with regulatory requirements pertaining to quality care delivery as well as in response to identified patient needs; promotes opportunities for input from persons served and other constituencies with regard to quality improvement issues. Will also provide comprehensive and effective response to any identified deficiency in collaboration with Quality/Compliance leadership, as well as assist in required clinical record reviews, and participate in all QI activities. Serves as an integral member of the senior management team, as well as key staff leadership for quality activities. This position will establish positive relationships and provide ongoing education with referral sources, regulatory entities, agencies, organizations, and individuals involved in patient care delivery as well as caregiver support. Overseas and assures compliance with staff education requirements and NYS approved training curriculums.
Educational Requirements:

Per regulation:
Hospice nurse coordinator shall mean a registered professional nurse who is currently licensed and registered in the State of New York and (i) possesses a baccalaureate degree in nursing and has a minimum of four years of professional experience in the delivery of nursing services which shall include at least two years in a supervisory or administrative position, or (ii) has a minimum of six years of professional experience in the delivery of nursing services which shall include at least two years in a supervisory or administrative position.
