On Time Talent Solutions is seeking a registered nurse with previous experience as a diabetes educator!<\/span><\/span>

  <\/div>
Diabetes Educator \- RN Summary:<\/span><\/span><\/strong>
<\/span><\/span> <\/div>
  • Provide diabetes self\-management education and diabetes management recommendations to patients and families in the in\-patient and out\-patient settings<\/span>
    <\/span><\/span><\/li>
  • Provide education \/ management to patients with all types of diabetes<\/span>
    <\/span><\/span><\/li>
  • Assess and develop a plan of care, evaluate effectiveness of plan<\/span>
    <\/span><\/span><\/li>
  • Works closely with the entire medical staff and nursing staff to prepare patients for discharge in a safe, efficient manner<\/span>
    <\/span><\/span><\/li>
  • Provides tools and skills for home care to patients and families. Makes recommendations to hospital staff for changes in diabetes management<\/span>
    <\/span><\/span><\/li>
  • Serves as an educational and informational resource to all hospital staff and the community.<\/span>
    <\/span><\/span><\/li>
  • Collaborate with medical, nursing, and case management staff<\/span>
    <\/span><\/span><\/li>
  • Remain current on the rapidly changing field of diabetes management and the latest recommendation for patient education techniques<\/span>
    <\/span><\/span><\/li>
  • Teach all aspects on insulin management
    <\/span><\/span><\/li> <\/ul>
    Diabetes Educator \- RN Qualifications:<\/span><\/span><\/strong>
    <\/span><\/span> <\/div>
    • Graduate of a Nursing program or Bachelors of Science in applicable field.<\/span>
      <\/span><\/span><\/li>
    • Two (2) years of experience in a clinical setting<\/span>
      <\/span><\/span><\/li>
    • Current License as a Registered Nurse \- RN or Certified Diabetes Educator \- CDE required<\/span>
      <\/span><\/span><\/li>
    • Experience in Diabetes Education preferred<\/span><\/span><\/li> <\/ul>
        <\/div><\/span>
