π About the Role
PanCare of Florida is seeking a knowledgeable, compassionate Diabetes Educator to support patients in improving their health through diabetes self-management education (DSME/T). In this role, you will empower individuals with the skills, tools, and confidence needed to manage diabetes and make long-term lifestyle changes.
Youβll work closely with patients, caregivers, and the healthcare team to ensure each individual receives personalized education, meaningful support, and ongoing follow-up. As a Diabetes Educator, you will:
Conduct DSME/T participant assessments using the AADE7 framework
Develop measurable goals, teaching plans, and individualized learning objectives
Provide education using interactive and skill-based methods to increase engagement and understanding
Document patient assessments, learning plans, progress, and evaluation outcomes
Perform follow-up assessments and track participant progress toward defined goals
Refer patients to appropriate healthcare providers when needs exceed scope
Communicate relevant findings and updates to the primary care provider and healthcare team
Participate in developing program and teaching materials
Support program accreditation requirements and participate in accreditation surveys
Engage in continuous quality improvement initiatives for DSMES service delivery
Track outcomes and assist in analyzing data to support value-based care goals
Participate in peer review, evaluation processes, and team education
Maintain required annual continuing education specific to diabetes and behavioral change strategies
Stay current with advancements in diabetes management, technology, and educational best practices

Confident presenting educational content to individuals and groups
Skilled in using computers, basic tech tools, and documentation systems
Strong communication and teaching skills
Active RN or LPN license required
Certified Diabetes Educator (CDE) or experience working in diabetes education strongly preferred
Experience with DSME/T programs required
Knowledge of the AADE7 Self-Care Behaviorsβ’ framework required
This role may require:
Sitting, standing, or walking for extended periods
Occasional bending, kneeling, or crouching
Lifting up to 40 lbs. (equipment, educational materials, or supplies)
Clear verbal communication and ability to observe non-verbal patient cues
Professional, calm demeanor when interacting with patients of all ages
