JOB SUMMARY:
Provides direct patient education to diabetic patients and families in a variety of inpatient and outpatient practice settings. Works as part of a multidisciplinary diabetic education team. Is cross-trained to work with other nurse educators to provide other types of patient and staff education, including CNA orientation, Cardiac (including heart attack, Congestive Heart Failure (CHF), bypass surgery, stent placement, and pacemaker), G-tube care, central line care, ostomy care, Lovenox, Coumadin, Stroke and other miscellaneous inpatient teachings as needed. Will teach in accordance to the American Diabetic Association (ADA) and American Association of Diabetic Educators (AADE) guidelines.
JOB DUTIES AND RESPONSIBILITIES:
Job Specific:
1. Facilitates/provides interdisciplinary diabetic teaching to patients based on assessed needs. (EF)
2. Facilitates/provides outpatient diabetes education classes and support groups. (EF)
3. Provides appropriate documentation of patient teaching. (EF) .
4. Promotes patient continuum of care by appropriate use of floor nurses/discharge planners/social services and
other departments and outside agencies. (EF)
5. Cross-trains to provide cardiac and other types of frequently requested patient teaching. (EF)
6. Participates in staff education as required (i.e. CNA orientation). (EF)
7. Participates in community events such as health fairs and public education. (EF)
8. Responsible for annual review and update of patient education materials. (EF)
9. Teaches Victoria College (VC) college students on the Meditech Expanse system and glucometers. (EF)
10. Cross-trained to work in the Employee Health clinic as needed. (EF)

EDUCATIONAL REQUIREMENTS
Required: Graduate of an accredited college, university, or school of nursing.

EXPERIENCE
Required: 2 years of clinical experience

LICENSURE / CERTIFICATION REQUIREMENTS
Required: Current license or permit to practice as a Registered Nurse in the State of Texas
