Provides proficient education for patients and families participating in diabetes self-management education program which promotes patient safety at home, school, and in the community. Collaborates and partners with other providers to develop and initiate an education plan for individual and group classes in an efficient manner that is meaningful to the patient and family.
Experience

Preferred Qualifications
- 2 years of experience in pediatrics
- Certified Diabetes Educator (CDE)
Education
- Bachelor's degree in Nutrition or graduation from an accredited nursing school (Bachelor of Science in Nursing for nurses)
Certification Summary
- Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact or Registered Dietician license
- Certified Diabetes Educator (CDE) as soon required number of diabetes education hours (1,000) is acquired and within 1 year of eligibility (must obtain minimum number of continuing education hours annually as required for American Diabetes Association recognition if not certified)
- Cardiopulmonary Resuscitation (CPR) within 30 days of employment
- Yearly certification in use of hospital-based blood glucose monitoring system and ketone monitoring
Knowledge, Skills, and Abilities
- Excellent communication, organizational, and interpersonal skills
- Good basic computer skills, e.g., understands and uses Microsoft Outlook, PowerPoint and Word and willing to learn Access and Excel
Job Responsibilities
Education
- Delivers family-centered diabetes education to patients ages newborn to 21 years.
- Determines appropriate educational plan for family and patient based on learning assessment with consideration for growth and development, psychosocial, and emotional factors.
- Provides diabetes support and education to hospital staff.
- Demonstrates expert knowledge of diabetes by providing current, fact-based information.
- Provides education that reflects the National Standards for Diabetes Self-Management Education.
Certification and Training
- Maintains and enhances knowledge of current diabetes self-management practices and helps to implement changes in diabetes program as standards of practice change.
- Meets all in-service requirements as outlined by human resources requirements and maintains competencies in job-specific tasks, including blood glucose monitoring on hospital system and ketone monitoring.
Outreach
- Assists in community outreach, e.g., walks/functions, speaking engagements, school/day-care training.
Administrative
- Assists in developing curriculum and implementing policies and procedures that guide and support the diabetes program and the system.
- Acts flexible with scheduling to meet departmental needs, including cross campus coverage on a regular basis.
- Handles conflicting demands on time and is able to prioritize issues efficiently.
