Acts as a coordinator of the Pediatric and Adult Diabetes Program, providing self-management education and resources for patients across the continuum, guiding them and their caregivers toward a quality outcome.Three to five (3-5) years of current hospital experience in clinical area of responsibility. Previous management experience is desirable; case management experience is a plus. Position requires positive interpersonal communications skills and outstanding verbal and written skills. Incumbent demonstrates effective organization, prioritization, and leadership skills and must be knowledgeable of the age specific physiological, psychosocial, developmental, and spiritual needs of the patient population typically served.Incumbent must be able to stand, and walk, for extended periods; may be required to bend and lift and have access to all areas of the hospital including detached buildings. Position requires excellent speech, auditory and visual acuity for communication of patient needs and observation of patients. Position my involve skin, eye, mucous membrane, or parental contact with blood or other potentially infectious materials.Required Licenses and Certifications
