The Redding Rancheria is seeking a Diabetes Educator. The preferred schedule for this position is 4-10 hour shifts, 7:00am-6:00pm for days between Monday through Friday.

JOB SUMMARY:

Provides individualized diabetes self-management education/ training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators and SDPI Guidelines. Follows established policies and procedures of the medical department and community health department.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. A healthcare professional who is also a certified diabetes educator, or who is eligible to become a certified diabetes educator, BSN or higher preferred
  2. Minimum of 6 months experience (preferably more) providing diabetes education within past year in diabetes and diabetes clinical/educational subjects
  3. CPR certification
  4. Must pass pre-employment drug screening
  5. Indian preference
  6. Must be fingerprinted and pass background check.

ESSENTIAL JOB DUTIES:

1. Assess needs of patients with diabetes and pre-diabetes; plan and implement programs and classes to meet these needs in conjunction with the services of the Community Health Director.

2. Documents all encounters and procedures in patient electronic medical record.

3. Collects DSME/T participant assessment data, using the appropriate framework, in a collaborative and ongoing manner.

4. Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.

5. Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.

6. Collaboratively develops an individualized follow-up plan with each program participant.

7. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.

8. Conducts a follow-up assessment upon completion of program services, using outcome measures from the Continuum of Outcomes Framework.

9. Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.

10. Utilizes a team approach to provide services and collaborates and communicates with team members when needed.

11. Identifies when a program participant’s needs are outside the scope of the instructor’s practice and expertise, plus arranges for additional services to meet needs.

12. Care of patients may additionally require home visits.
