Champion Diabetes Care in Aged & Community Services

At Lutheran Services, we believe diabetes care should be as compassionate as it is expert. As our Credentialled Diabetes Educator (CDE), you’ll play a vital role in enhancing the health and wellbeing of older adults living with diabetes across our residential aged care and community services. You’ll provide expert education, clinical support, and care coordination—ensuring safe, evidence-based diabetes management that uplifts lives and aligns with aged care reform priorities.

Location: Support Centre with frequent travel to sites.
Employment Type: Permanent Part-Time 0.8 FTE.
Remuneration: Circa $135K (FTE) + Superannuation + Vehicle Allowance

About The Role

As a Credentialled Diabetes Educator, you’ll lead with empathy and expertise, championing our values of kindness, integrity, innovation, and empowerment. You will:

  • Provide expert diabetes education and clinical guidance to residents, clients, families, and staff
  • Develop and support individualised diabetes care plans in collaboration with care teams
  • Deliver ongoing training to staff and tailored education to residents and families
  • Ensure compliance with aged care standards and contribute to audits and quality improvement
  • Coordinate care with external health professionals and support aged care reform initiatives

This role involves frequent travel and occasional overnight stays. Vehicle Allowance Available. 

About You

You’re a confident and compassionate clinical educator who brings:

  • A recognised nursing qualification and current AHPRA registration
  • Credentialled Diabetes Educator (CDE) status via ADEA
  • Strong knowledge of aged care standards and chronic disease management
  • Proven ability to deliver engaging education and collaborate across teams
  • A valid QLD Driver’s Licence, NDIS Worker Screening clearance, and willingness to travel
  • Experience developing and delivering interactive training programs
