The Community Health Educator will play a vital role in a community-based public health project aimed at reducing the prevalence and impact of hypertension. This position involves developing, implementing, and evaluating health education programs that promote awareness, prevention, early detection, and effective management of hypertension, especially among high-risk populations. 

 

As part of the essential functions of this role, The Community Health Educator

  • Conducts outreach to raise awareness about hypertension and cardiovascular disease 
  • Assists with the development of the annual outreach plan, which includes face-to-face and social media strategies, goals, and objectives for reaching the target population 
  • Facilitates community outreach efforts to identify and engage individuals at risk for or currently managing hypertension, through collaboration with local clinics, community centers, faith-based organizations & other stakeholders  
  • Assists with production of promotional items, brochures, fliers and other program literature that support and enhance outreach strategies 
  • Collects and supports client collection of necessary data to monitor health goals and outcomes for each client, including but not limited to blood pressure readings, medication adherence and behavior change metrics 
  • Provides interventions and education to increase adherence to medications, nutrition care, and medical appointments 


Qualified candidate will have a High School diploma or equivalent, Experience in community health education or outreach, preferably with a focus on chronic disease prevention; Knowledge of cardiovascular health education preferred; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
