Position Summary
The Clinical Support Specialist plays a vital role in supporting the clinical operations of a home health agency. This position provides direct assistance to the clinical team, helping ensure accurate documentation, regulatory compliance, and effective coordination of patient care. The ideal candidate is detail-oriented, tech-savvy, and experienced in home health workflows, particularly related to EMR systems, OASIS documentation, and scheduling support.
Key Responsibilities
Clinical Documentation & EMR Support
- Review and audit clinician documentation for accuracy and completeness (e.g., OASIS, Plan of Care, visit notes).
- Track, flag, and follow up on documentation deficiencies or missing signatures.
- Assist with uploading, organizing, and maintaining accurate clinical records in the EMR system.
Coordination & Communication
- Support field staff with real-time clinical or scheduling questions.
- Serve as liaison between clinicians, intake, QA, billing, and administrative departments.
- Communicate with physicians' offices to obtain signed orders and clarify care plans.
Regulatory & Compliance Support
- Monitor compliance with Medicare Conditions of Participation and agency protocols.
- Assist in preparing for surveys or audits by ensuring clinical records are complete and accurate.
- Support the QAPI program through data collection and tracking of clinical indicators.
Training & Technical Support
- Provide EMR guidance to new or existing field staff.
- Assist with onboarding processes for new clinicians regarding documentation workflows.
Qualifications
Education & Licensure:
- High school diploma or equivalent (required); associate’s or bachelor’s degree in health sciences, nursing, or related field (preferred).
- LPN Required
Experience:
- 1–2 years of experience in home health, healthcare administration, or medical office setting.
- Strong working knowledge of EMR systems
- Familiarity with OASIS, Plans of Care, Medicare documentation standards a strong plus.
Skills:
- Excellent organizational and communication skills.
- Strong attention to detail and ability to multitask in a fast-paced environment.
- Proficiency in Microsoft Office (Excel, Outlook, Word).
- Ability to work both independently and as part of a team.

Working Conditions
