Clinical Support Licensed Practical Nurse (LPN)

Location: Keystone Family Medicine—Fifth Avenue
Schedule: Monday-Friday | Full-Time (40 hours/week) | No weekends or holidays
 
Keystone Family Medicine is seeking a compassionate, detail-oriented Clinical Support LPN to join our fast-paced, patient-centered team. In this role, you’ll provide direct patient care and assist our medical providers to ensure efficient, high-quality service for every patient. We are looking for an LPN who thrives in a collaborative environment, communicates effectively, and upholds our commitment to exceptional healthcare.

Key Responsibilities

• Provide patient care within the scope of a Licensed Practical Nurse (LPN), maintaining all required competencies.
• Prepare for patient appointments by reviewing records, reconciling medications, and collecting vitals.
• Assist providers with exams, procedures, and follow-up care.
• Carry out provider orders, including lab requisitions, imaging requests, and medication administration.
• Educate patients on treatment plans, test results, and preventive care.
• Collect and prepare lab specimens.
• Maintain exam room readiness, including cleaning and sterilizing equipment.
• Document accurately and promptly in the Electronic Health Record (EHR).
• Participate in team meetings, required trainings, and quality improvement initiatives.
• Maintain confidentiality and adhere to HIPAA regulations.
  
  

Qualifications

• Graduate of an accredited nursing program.
• Current Pennsylvania LPN license.
• Current CPR certification.
• Minimum one-year experience in a clinical setting.
• Proficient in Microsoft Office and EHR systems; EPIC experience preferred.
• Strong verbal and written communication skills; Spanish proficiency preferred.
• Exceptional time management, prioritization, and attention to detail.
