Position Summary: 

Directs the unit responsible for clinical evaluations and assessments, clinical updates, case formulations, and recommendations for extended acute, long term inpatient psychiatric care, and for individuals with severe and persistent mental health and co-occurring issues within the DBHIDS system as necessary. The unit is responsible for providing ongoing monitoring of these individuals while inpatient (extended acute, acute, or otherwise noted) to assure that adequate care and treatment is taking place in a recovery oriented environment.

Duties and Responsibilities: Administrative:

Administrate, update, and review annually the policy and procedures for referral, assessment and monitoring extended acute services.

Develop and monitor protocols for establishment of continuity of care among community mental health providers, community psychiatric inpatient hospitals (acute), extended acute care (EAC) psychiatric inpatient units, and other mental health providers as necessary. This includes but not limited to visiting with acute hospital (referral source) regularly (weekly), EAC regularly (monthly), and other mental health stake holders upon request to monitor and update procedures regarding the evaluations and assessments, referral, transfer, discharge process, and provide other supports as necessary.

Referred members approved for EAC by CBH will be monitored, via a preapproved screening protocol with CBH and various stakeholders (acute hospitals). Screenings should be completed within a timely manner, preferably within one business day, and documenting of said screenings will be kept and monitored in a spreadsheet for data reporting purposes.

ADAPT Assessments are to be forwarded to the appropriate stakeholders with continued oversight regarding acceptance into approved EACs. When there are concerns about approved admissions, continued participation in the referral process is required until an acceptable outcome is agreed upon, which may include but not limited to coordinating and participating in multiteam meetings.

Assist in facilitating necessary paperwork and transfers from acute to extended acute and extended acute to long term residences or long term inpatient (state hospital) as necessary.

Maintain Referral Lists for admissions and discharges for EACs and distributed to required stakeholders on a weekly basis.

Supervise professional staff and train as necessary. This includes but not limited to completing yearly staff evaluations within the required time frame as indicated by supervisor and/or HR, assisting with staff professional development, i.e. overview of professional and educational trainings, licensing progress, and self-identified work goals (short term and long term).

Requests for ADAPT Assessments from various stakeholders will be addressed within a timely manner, with the first reply being completed within 24 hours. Said documentation should be provided after all procedures have been completed and verified.

Data collection will be maintained with the latest tools available and as indicated by the department

Correspondence from various stakeholders will be responded to within one business day of initial contact.

Remain aware of current and relevant changes in the DBHIDS/PMHCC system and funding sources as they relate to ADAPT and DBHIDS/PMHCC.

Coordination of office equipment, resources, and supplies required to complete work tasks for team

Clinical:

Provide leadership and DBHIDS representation at staff meetings, clinical case conferences and team meetings at area hospitals. This includes but not limited to bi-monthly staff meetings to address teams clinical concerns and team cohesiveness and goals

Provide support to the team in regards to assistance with evaluations and assessments, including but not limited to completing evaluations and assessments, clinical updates, and case formulations as necessary.

Keep up to date with clinical changes within the industry by attending certified trainings

Maintain all clinical licensing and certifications

Other duties as defined by supervisor.

Skills Required: 

Leadership and mentoring experience, and ability to manage a diverse team with success

Excellent clinical, interviewing, assessment/evaluation skills.

Demonstrated skills in communication, interpersonal relationships and networking with stakeholders

Thorough knowledge of MS Word, Excel, and other computer programs available to team, i.e. MCO.

Experience in diagnostic screening and assessment, treatment planning utilization review

Knowledge of various mental health and co-occurring resources in Philadelphia, PA.

Knowledge of the DSM-5, mental status exam and multi-axial diagnostic system. Understanding of diagnostic criteria and appropriate treatment approaches, clinical guidelines and best practices.

Excellent writing ability

Ability to represent DBHIDS/PMHCC and ADAPT in a professional manner within the agency and with all stakeholders.

Education and Experience

Minimum of a master’s degree in psychology, clinical social work or human services discipline and a minimum of seven years clinical experience, at least five years in a public mental health setting. At least three years’ experience in administrative capacity. Licensing in above field of study is preferred. Valid driver’s license and accessibility to a vehicle
