The Location:
Kalele Care Services was founded in 2020 by Ohana Pacific Health to address the complex healthcare needs of the community with an emphasis on assisting Kupuna to live longer and healthier independent lives.
The Position:
Coordinates operations of care management services that bridge care across multiple settings and domains. Tracks and reports clinical and operational processes and outcomes. Directs and manages day-to-day Kālele Care Services (“Kālele”) operations, including directly supervising the Community Healthcare Workers (CHWs), Social Workers (LSWs), Nurse Care Managers (RN CMs), Office Coordinators, Schedulers, and any other program support personnel to ensure achievement of optimal outcomes through superior customer service, management of resources, and interdisciplinary collaboration. Trains and mentors staff, overseeing compliance with processes, documentation, billing, data inputs, and data reporting and analytics for key service line metrics. Works closely with Advanced Practice Providers (APPs) in ensuring optimal patient management throughout service duration. Provides social services support for patients on a consultative basis.
