Sangre de Cristo Community Care is seeking a dedicated and empathetic Clinical Liaison (Registered Nurse) to join our dynamic team and help us build strong relationships within the healthcare community while providing exceptional clinical support. The Clinical Liaison will serve as a pivotal link between our organization and the healthcare community. This role requires strong clinical skills to assess and understand patient needs and exceptional relationship-building skills to foster trust and collaboration with referral sources.

Responsibilities:
1. Graduate from an accredited school of nursing as an RN. BSN preferred; and current license in the state of Colorado.
2. Two years of experience working in an acute care setting; one year of hospice experience preferred. Working knowledge of community resources. Ability to meet, collaborate and work efficiently with others in the community. Knowledge of entitlement programs.
3. Must show Proof of a Tuberculin Skin Test (PPD), TB blood test, and/or CXR, or get one upon employment (An annual assessment for symptoms must be completed thereafter)
4. Must possess a valid driver’s license issued by the state of Colorado.
