Medical and Rehab Liaisons

The Clinical Liaison is responsible for establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns.

Essential Job Functions/Responsibilities

  • Works with the Director of Clinical Services to establish community education for clinical programs.
  • Establishes and maintains positive working relationships with current and potential referral and payer sources.
  • Assists referral sources with smooth transitions of care for patients receiving home health or hospice services.
  • Ensures that all necessary paperwork is obtained in order to admit new patients to Home Health or Hospice service.
  • Assists in the required coordination for admissions and transfers between Elite Home Health and Hospice and other community healthcare partners
  • Monitors the status of and follows-up on patients that are currently in a Hospital or other facility
  • Provides Hospice informationals to potential hospice patients, appropriate family members, POA and/or legal guardian.
  • Builds and monitors community, customer, and payer and patient perceptions of Elite Home Health and Hospice as a high quality provider of services.
  • Maintains comprehensive working knowledge of Elite Home Health and Hospice markets including government agencies, major payer groups, key referral sources, and competitor’s market positioning.
  • Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate organization personnel.
  • Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Elite Home Health and Hospice

Position Qualifications

  • Licensed as a Licensed Practical Nurse or Registered Nurse in Washington or Idaho. Must be able to obtain license in both states.
  • Associate or Bachelor's degree in Marketing, Business Administration, or related field preferred, but not required.
  • At least one year experience in health care marketing preferably in hospice care operations.
  • Demonstrates good communications skills, negotiation skills, and public relations skills.
  • Demonstrates ability to take initiative to accomplish agreed upon goals.

Salary: $75,000 - $85,000 annually
