Description

The Clinical Liaison is responsible for supporting strategic growth, referral development, and patient access across HealthBridge Children’s Hospital and affiliated programs within the Southern California region. This role serves as a key clinical connection between referring hospitals, providers, and HealthBridge, helping ensure appropriate patient placement, smooth transitions of care, and strong ongoing referral relationships.

This position combines clinical expertise with relationship-building skills to evaluate referrals, communicate clinical appropriateness, and support timely admissions. The Clinical Liaison plays an essential role in expanding awareness of HealthBridge services and ensuring medically complex pediatric patients receive the right level of care at the right time.

The ideal candidate brings a strong clinical background—preferably as a Registered Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist—with experience in post-acute care, rehabilitation, or pediatric settings. This clinical foundation is essential to understanding patient needs and effectively partnering with physicians, case managers, and hospital teams.

Key Responsibilities

Referral Development & Clinical Review

  • Serve as the primary clinical point of contact for referral sources, including acute care hospitals, physicians, case managers, and payors
  • Review and assess clinical referrals for appropriateness based on HealthBridge admission criteria
  • Communicate clinical recommendations and coordinate timely admissions with internal teams
  • Support seamless patient transitions from acute care to post-acute and rehabilitation settings

Relationship Management

  • Build and maintain strong professional relationships with key referral partners across the region
  • Act as a liaison between external providers and internal clinical/admissions teams to ensure clear communication and high-quality service
  • Represent HealthBridge in regional hospital meetings, discharge planning rounds, and community outreach efforts

Market Awareness & Growth Support

  • Identify referral trends and potential barriers to patient placement
  • Provide feedback from referral sources to support service line development and operational improvements
  • Support awareness of HealthBridge programs through education and clinical presentations

Data Tracking & Reporting

  • Track referral activity, admissions, and conversion trends in collaboration with internal teams
  • Use data insights to support outreach efforts and improve referral responsiveness
  • Maintain accurate documentation of referral interactions and outcomes

Cross-Functional Collaboration

  • Partner closely with admissions, nursing, therapy, case management, and leadership teams to ensure coordinated patient flow
  • Support development of clinical education materials for referral sources
  • Participate in strategic discussions related to access, capacity, and program development


Qualifications

Qualifications

  • Bachelor’s degree in Nursing, Rehabilitation, Healthcare Administration, or related field
  • Clinical licensure strongly preferred: RN, RRT, PT, OT, or SLP
  • Minimum 2 years of clinical experience; experience in clinical liaison, case management, or post-acute care strongly preferred
  • Strong understanding of hospital discharge planning and continuum of care
  • Excellent communication and relationship-building skills
  • Ability to assess clinical information and apply admission criteria appropriately
  • Comfortable working independently and traveling regionally as needed
  • Valid driver’s license required
