What does a Clinical Liaison Assessment Coordinator at Landmark Hospital do?

The Clinical Liaison Assessment Coordinator (CLAC) is responsible for supporting the smooth admissions of qualified patient referrals into our Long-Term Acute Care hospital.  They support the overall process by reviewing referred patient medical records and notes, validating criteria for admission, and confirming insurance coverages.  CLAC translates the patient information to the assessment forms to ensure that the patient fits within our services and admission criteria.  Collaborates with the business development specialists and admission teams to complete the referral and admissions process in a timely manner.  May conduct in person patient assessments at referring hospital locations as needed.


Essential Functions:

  • Synthesizes patient information from various sources (Field notes, Medical Records, etc.) to complete patient assessment forms
  • Uses assessment form, referral data and medical records to identify, validate and assist with evaluating the patient against criteria for admissions.
  • Reviews patient insurance information to confirm benefits and coverage by applying knowledge of insurance programs, hospital programs, regulations and criteria for admissions
  • Collaborates and maintains frequent communication with business development specialists and admissions department to gather missing data needed for assessment
  • Maintains and organizes status of referral pipeline with regard to assessment activities
  • May conduct patient assessments at referring hospitals as needed
  • Other duties as assigned

Required Education 

• High school diploma or equivalent (preferred) 

Required Experience 

• Clinical license or certification (preferred) 

Required Licensure / Certifications 

  • Drivers’ License 
  • Valid automobile insurance
