RESPONSIBILITIES 

Including, but not limited to: 

  • Manage and direct the admissions process with referrals which include hospitals, Doctors’ offices, nursing homes, and other related businesses according to established policies and procedures. 

  • Screen and accept referrals timely with the understanding that outliers are to be reviewed by the Administrator/DON. 

  • Generate new referral leads at assigned accounts. 

  • Convert potential referrals to admissions to ensure each facility can meet and exceed set targets and sales goals. 

  • Serve as a liaison between referrals, hospitals, physicians, and care professionals. 

  • Coordinate with nursing and housekeeping staff to update bed availability, and ensure rooms are clean, labeled, and ready for potential admissions. 

  • Communicate and coordinate admissions with the external liaison team and VP of Business Development. 

  • Ensure all admissions-related documents are uploaded into the medical records system. 

  • Maintain updated bed availability and daily census. 

  • Communicate special needs of new admissions to staff ensuring a smooth transition. 

  • Maintain a working knowledge of Federal and State regulations and reimbursement. 

  • Maintain knowledge of Medicare and Medicaid regulations. 

  • The ideal candidate will also have previous organizational experience. 

  • All other duties as assigned.

SKILLS

  • High school diploma or equivalent 

  • Previous experience in long-term care or other healthcare setting is a plus. 

  • Detailed oriented with excellent follow up skills. 

  • Ability to multi-task and work in a fast-paced environment. 

  • Excellent interpersonal skills. 

  • Knowledge of office technology 

  • Comfortability with Microsoft Office. 

  • Ability to learn new software programs. 

  • Previous experience in a skilled nursing facility.  

  • Must be capable of maintaining regular attendance 

  • Good verbal and written communication skills 

  • Must be able to maintain confidentiality regarding employee, resident and company 

  • proprietary information 

  • Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels 

  • Must possess computer skills of the types and at a level necessary to accomplish the job.
