Purpose of Your Job Position
The primary purpose of your job position is:
- To evaluate and assess potential patients for clinical appropriateness, sub-acute potentially high risk and managed care referrals. Communicates with Director of Nursing/Admission Director regarding special patient needs, physician and/or Insurance Case Manager objectives/plan and patient/family dynamics and expectations.
- To provide clients (prospective residents and their families and referral sources) with facility and program specific information via community outreach and networking, facility tours, personal visits and assessments presentations and follow up.
- To conduct business development efforts that drive awareness of facility and services and generate referrals and admissions to the facility.
- To assist with directing the admissions process coordinating admission documentation.
- To participate in the facility-specific sales and marketing team in development and implementation of the marketing/sales plan.
- To continually monitor the needs and service satisfaction levels within the community and to alert the facility management staff of new or changing demands.

Duties and Responsibilities:
- Regular, punctual attendance with flexible hours as clinical liaisonβs responsibilities dictate.
- Responsible for receiving referrals and inquiries from hospitals, families, and others. Comply with all state and federal requirements for admission.
- Conduct pre-admission evaluations whenever necessary to ensure appropriate placement of the patient or resident.
- Make person-to-person calls outside the facility to medical professionals, senior organizations, appropriate special interest groups, hospital discharge planners and other community contacts. Develop and maintain relationships, advise them of bed availability, new products and services and other pertinent information.
- Alert appropriate department managers and facility team of projected admissions. Maintain a close working relationship with the team to assure appropriate and efficient decisions about prospective admissions. When admission determinations are made, work with team on appropriate room and bed selection.
- Coordinate appropriate admission paperwork and obtain signatures from patients or responsible parties prior to admission in a manner consistent with state and federal regulations. Communicate special needs of new admits to staff to ensure an efficient admission. Communicate admissions decision in a timely manner. Refer denials to appropriate sister facilities as necessary.
- Maintain a working knowledge of federal and state regulations and reimbursement (Medicare and Medicaid).Obtain appropriate financial information prior to the admission.
- Monitor inquiry, admission and census trends. Maintain inquiry/referral log and other reports required. Monitor all active and inactive inquiries.
- Maintain a database of medical contacts and community resources along with knowledge of industry trends; communicate this information with facility staff and community contacts.
- Monitor and evaluate customer satisfaction.
- Assist SR VP of Business Development in the development of facility materials like brochures, press releases that describe the facilities services as necessary.
- Serve as a member of committees whose purpose is to enhance resident and community relations, attend weekly meetings assist in implementation of sales and marketing plans, monitor budgets and track results.
- Develop and implement special events and presentations targeted at community education, establishing and maintaining status for the facility as the expert on skilled nursing and rehabilitation care in the community. Serves as a community resource and maintain contacts for information exchange.
- Other duties as assigned.
Education:
Must possess a Bachelorβs degree.
Specific Requirements/Skills:
- Ability to perform clinical assessments (preferably a RN or LPN) using critical thinking skills in developing potential care plan, estimated length of stay, patient/family expectations and recommend appropriate level of care within the facility and taking into account the patients specific financial platform.
- Knowledge of (or ability to learn) reimbursement, clinical criteria specialty programs and payer source for all levels of care offered in the facility.
- Must be able to read, write, speak and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, visitors and the general public.
- Organized and detailed in work performance.
- Good communication skills with excellent self-discipline and patience.
- Genuine caring for and interest in elderly and disabled people in a nursing facility.
- Comply with the Residentsβ Rights and Facility Policies and Procedures.
- Perform work tasks within the physical demand requirements as outlined below.
- Perform Essential Duties as outlined above
